South Lake Pediatrics is proud to be certified to be a Health Care Home by the Minnesota Department of Health since May, 2013. A centerpiece of Minnesota Health Reform Initiative, a renewed focus was placed on the importance of a primary care relationship.
Health Care Home is a philosophy of delivering health care. It does not represent a physical building; rather it is a team approach to care that gives you access to all the services and support you need. The Health Care Home is about patients and families becoming partners with the Health Care Home team. This team is comprised of our Pediatricians, Pediatric Nurse Practitioners, Care Coordinators and other support staff. Involving patients and families in all aspects of the clinic experience is how this happens. When patients require referrals to a specialist or specialized treatment, all options are explained to them and together care decisions are made. Everyone benefits from coordinated, patient and family-centered care; however, a Health Care Home is especially important for people with chronic conditions such as asthma, autism or depression.
