Required Forms After Turning 18:
This form informs the patient of their rights and responsibilities while receiving care at South Lake Pediatrics. It must be completed and verified annually.
At age 18, the patient must grant permission for a parent or guardian to be involved in their healthcare. This includes scheduling appointments, requesting and picking up prescriptions, and accessing mental health care information (if applicable). This consent needs to be signed annually.
This form grants permission for South Lake Pediatrics to send automated, artificial voice and/or prerecorded messages, voicemails, text messages, and electronic mail about health care and related matters. This consent only needs to be completed once.
When a patient turns 18, they are responsible for the payment of their medical bills. This means that billing statements will be issued in their name. If the patient wishes to remain under the current guarantor’s account (e.g., their parent or guardian), written consent from the current guarantor is required. This form only needs to be completed once.
Important Notes:
- Forms must be signed on or after the patient’s 18th birthday. Any forms signed before this date will be considered invalid, and new forms will need to be completed.

Patient Portal Access:
After turning 18, the patient’s current Patient Portal login will expire. The patient can choose to reactivate their portal account by calling 952-401-8300. If the patient wishes to grant their parent or guardian access, they will need to share the new login information.
