First, you’ll want to transfer your medical records to us. For patient privacy reasons, we are not able to transfer records for you; only you can officially transfer your medical records. Please use this Release of Medical Information Form. Once completed, please send it to your previous clinic and they’ll transfer your records to us.

Also, please print out and complete the Demographic Form, Consent for Services Form, and Family Health History Form. Bring the completed forms to your first appointment.  Please bring your health insurance card as well.

Now, just call our appointment line at 952-401-8300. We’ll be sure to schedule the right amount of time with the health care provider of your choice, on a day that works best for you.
