Care Coordinators supports patients and families dealing with chronic or complex health conditions, developmental issues, mental health and social needs.
Care Coordinators can help identify and refer to community resources; including mental health, community services and social services. We coordinate care through a team approach. The team includes you and your family, your primary South Lake Pediatrics Clinician, a Care Coordinator as well as other team members outside of your health care home. Your team identifies and assists you in accessing the medical and community services you need to reach your best medical, social, economic and psychological health. Care Coordinators also work with patients and families to set goals and in some cases develop care plans.
