We welcome new patients to the South Lake family and look forward to providing care to newborns and those new to the neighborhood.

Due to the HIPPA privacy act, we are not permitted to transfer your medical records to South Lake Pediatrics for you; only you can officially transfer your records. Please use our Release of Records Form. Once completed, please send it to your previous clinic and they’ll transfer your records to us.

Also, please bring our Demographic Form and your health insurance card to your first appointment with us.
